Welcome to Back-In-Action new patient center!

We understand that arriving at a doctor's office for the first time can be nerve racking. Filling out paperwork, learning  new faces and names, and getting to know your way around the office can be an overwhelming experience.

That's why at Back-In-Action our aim is to  remove stress, avoid unnecessary tension and make you comfortable as you fill out your paperwork.

If you prefer, in this area of the website you will be able to take care of items necessary for your visit prior to arriving, in the comfort of your own home.  Simply print out your required paperwork, fill it out, and bring it with to your appointment.  If you'd rather complete the paperwork at your appointment time,  please arrive 15 minutes early, and we will have the paperwork ready for you.  In addition to the paperwork, we will need a copy of your insurance card and a photo identification at your first appointment.
